The Requirements

To meet the basic qualifications for this role you will have legal authorization to work permanently in the United States for any employer without requiring a visa transfer or visa sponsorship. To be a good fit for the Primary Care RN opportunity, you also should have:
  • A bachelor's degree in Nursing from a four-year college or university
  • Washington State Registered Nurse license
  • Current CPR and BLS certification
  • A passion for collaborative and team-based care, and strong motivation to work to the top of the nurse scope of practice
  • A commitment to patient-centered care and to building ongoing therapeutic relationships with individuals of varying backgrounds, ethnicities, socio-economic levels and cultures
  • Willingness to provide patient care within the care center, patient home or other facilities as needed
  • A valid driver's license and insurable DMV record

The Role

Reporting to the Practice Manager, you will join a new team that includes one other Primary Care RN; we expect the team and program to grow. Your primary focus will be to optimize the health status of our patients. This will include utilizing the nursing process within a patient-centered and team-based care model to achieve Concerto's mission. You'll provide chronic disease self-management support, patient education, emergent triage, and complex wound care.

You'll invest the majority of your time in supporting a panel of patients with complex needs, including assessing needs, understanding the social context, and helping the team determine the best ways to help those patients achieve goals. In addition, you may spend about 15% of your time performing advanced telephonic triage, as well as conducting home visits, often related to wound care.

More specific activities will fall into three different areas. Working in partnership and joint accountability with care teams, you will:
  • Manage a defined panel of patients and work as a member of ConcertoHealth's care teams
  • Utilize the electronic health record to communicate with members of the care teams and implement care plans in person or by telephone according to Concerto's Health Tasking Guidelines
  • Work with others to assess and redefine priorities as needed, and support colleagues throughout the organization either on site or remotely
  • Work in partnership with care team members to plan and communicate care delivery, including delegating to LPN, MA and PSR team members as appropriate
With regard to chronic disease management and patient education, you will:
  • Identify patient goals for improving their health status and management of diseases
  • Assess patient understanding of their disease and readiness for change
  • Provide patient with education about the disease process and offer tools and skills needed to achieve self-management goals
  • Promote wellness and the importance of preventive care
  • Provide motivational interviewing and harm reduction counseling around health behaviors
  • Educate patients and families/caregivers in the use of medications and lifestyle changes in therapeutic management of acute and chronic health conditions
  • Educate patients and families/caregivers regarding abnormal laboratory results requiring education and follow-up
In the area of triaging emergent health issues and performing select clinical procedures, you will:
  • Perform telephone or in-person triage and home care advice for patients in accordance with Telephone Triage Protocol references
  • Administer select injections and medications
  • Provide complex wound assessment, debridement, and dressings utilizing modern moist wound care best practices as appropriate by training, licensure and Concerto protocol and policy
  • Perform and assist with procedures as appropriate by licensure, training and Concerto protocol and policy
