The Requirements
To meet the basic qualifications for this role you will have legal authorization to work permanently in the United States for any employer without requiring a visa transfer or visa sponsorship. In addition, you will have:- Licensed Independent Clinical Social Worker or Licensed Mental Health Counselor (LMHC), license to practice independently in the State of Washington.
- Master’s degree or doctoral degree in Social Work, Counseling or another behavioral health field.
- Solid understanding of behavioral medicine and evidence-based treatments for medical and mental health conditions.
- Competency in cognitive behavioral therapy and motivational interviewing preferred.
- Experience working in integrated medical settings preferred.
- Ability to multi-task and stay fluid with ever-changing needs in a dynamic environment.
- Experience with electronic health record systems.

- Impeccable interpersonal and communication skills.
- Ability to travel as needed in a timely manner within Renton & Lakewood to locations that may have limited access to public transportation; private vehicle and proof of liability and property damage insurance on vehicle is required; Licensed driver for a minimum of 3 years with no violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony driving within 7 years. The Role
Reporting to the Social Services Supervisor, you'll be an integral part of the Patient Engagement Team, addressing the needs of our most medically and socially complex patients. Your mission is to provide a combination of evidence-based individual and group therapies and counseling to improve patient outcomes. Our overall goal for this program is to reduce emergency room and hospital admissions by 20% and we'll look to you to fully align with and meet/or exceed this goal.Concerto's general patient population encompasses medically, mentally and socially challenged, complex and vulnerable cases, but we also have a high-risk, high-utilizer group that we call "Hot Spotters". Quick question for you - click here You'll spend 50% of your time with counseling and case management of "hot spotters"; another 40% counseling the general patient population; and the remaining 10% with outreach, visiting any of your patients that end up in the Emergency Room or hospital. You'll focus on short-term counseling and then transition those patients to other community-based counselors. You'll rotate between our Renton (2 days) and Lakewood (3 days) offices that are 40 minutes apart drive time.
Specifically, your responsibilities will include:
- Providing brief, focused individual therapy in collaboration with a multi-disciplinary team.
- Evaluating crisis situations and employing appropriate interventions.
- Improving population health outcomes by ensuring all patients and primary care team members have access to behavioral health expertise.
- Facilitating and overseeing referrals to specialty mental health services and supporting a smooth transition from primary care to specialty mental health services.
- Facilitating group-based visits and workshops to holistically address medical and behavioral health needs.
- Providing education and consultation to providers and other team members on the principles of brief behavioral and cognitive behavioral interventions and reinforcing their use in medical visits.
- Attending scheduled multi-disciplinary team meetings and providing relevant recommendations to improve the health of high-risk patients and those who have recently been hospitalized.
