The Requirements
To meet the basic qualification for this role, you will have legal authorization to work permanently in the United States for any employer without requiring a visa transfer or visa sponsorship. In addition, to be a good fit for the Physician Assistant / Advanced Practice Registered Nurse opportunity, you will have:- A degree from an accredited health professions school
- Board certification as a Physician Assistant or Advanced Practice Registered Nurse

- 2 to 3 years of experience in outpatient nursing, including:
- Diagnosing and treating the wide range of medical issues seen in the adult population including complex cases with multiple co-morbidities
- Skill in providing care for the whole person, including patients living with mental illness and substance use disorders
- Experience working and coordinating care with internal members of the care team as well as external providers
- A desire to work as advocate for your patients
- Average to advanced computer and software skills (Microsoft Word, Excel, Outlook)
- Training and/or experience with home visits, geriatric medicine, or palliative care is strongly preferred
- Experience in Federally Qualified Health Centers (FQHCs) and/or community health programs (CHPs) is a plus
The Role
Reporting to the Lead Physician, Field Team, you will provide field-based care to members in Kalamazoo County and parts of two adjacent counties. Quick question for you - click here The Kalamazoo offices opened in June 2017, and our innovative team is committed to improving care for underserved and complex-needs patients.We place a high value on collaboration, and you will participate in integrated care plans and partner with multidisciplinary team members, including nurse care managers, clinical social workers, clinical pharmacists, behavioral health specialists and others.
Your caseload will vary according to the complexity of cases, but once you are up to speed you may carry 40 to 50 cases. You'll make five or six home visits a day, seeing each patient about once a month. Approximately 20% of clinical time is provided for administrative duties. A typical day will start with a team huddle, either in the office or virtually, and on some days you may travel with another team member.
Our schedules allow providers to spend extended amounts of time with patients so they can focus on relationship-based care, which is key to our care model. In addition to providing direct care, you'll work with the team to bring close attention to the social determinants of health and an intense focus on transitions of care, always seeking to identify root etiologies for avoidable utilization of the health care system.
